How to Prepare for Your Appointment
To help you prepare for your visit, please review the information below based on your appointment type.

Please ALWAYS bring a current insurance card with you to your visit. Plans are often updated or slightly modified especially at the beginning of a year. We will need to check your card each visit for any changes and scan if needed. Incorrect insurance billing information can cause delays in processing your claim and cause charges to be transferred to you until correct information is obtained.
New Patient Appointments
Includes: New patients and those returning after 3+ years
Please arrive 10 minutes early for the check-in.
Bring your insurance card and photo ID (parent/guardian ID if patient is a minor).
If testing is scheduled, do not take antihistamines or high doses of vitamin C (>1000mg per day) for 5 days before your appointment.

Follow-Up Appointments
Please arrive 10 minutes early for check-in and bring any questions for your provider.

Rush Immunotherapy Appointments
Please arrive 10 minutes early for check-in.
Your appointment will last approximately 6 hours.
Bring your unexpired EpiPen, Pre-RUSH medications, snacks, and drinks.
Please make sure you have followed the RUSH pretreatment medication plan starting 2 days prior to your appointment exactly as outlined. Take your pretreatment medications on the day of your appointment at least 1-2 hours prior to arriving at 8 a.m.

Appointment with Provider + Allergy SHOT
Please arrive 10 minutes early for check-in and bring any questions for your provider.
If you are planning to receive your allergy shot at your visit, please prepare as you would for your allergy shot only visits - if you are required to take an antihistamine or bring an unexpired Epipen to your allergy shots, this will also be required for this visit.
If you have questions, please call 503-228-0155 and ask for one of the nurses.
Please note: It is not possible to do allergy testing AND receive an allergy shot on the same day.

Monitored Challenge Appointments
Includes: Oral Food Challenges (OFC) and Medication Challenges
Please arrive 10 minutes early for check-in.

FOODS: Please bring 1-2 full servings of the food to be challenged with you. If it requires home preparation, please prepare as simply as possible (limited spices/other foods).
MEDICATIONS: you will likely need to pick up the medication to be challenged at the pharmacy ahead of time. If you are unsure, please call our office to clarify. If the medication to be challenged is an over-the-counter medication, please bring the entire bottle with you to the appointment including the original label.

SLIT (sublingual immunotherapy) - Grastek/odactra
Please arrive 10 minutes early for check-in.
Please bring your unexpired EpiPen. If you need help accessing an affordable Epipen prior to your appointment, please let the nurses here at the clinic know.
Please continue your normal allergy and asthma medications (including antihistamines if you are taking them). Your allergy symptoms need to be relatively well controlled prior to starting SLIT.
This year, we are having patients sign an Advanced Beneficiary Notice (ABN) before administering SLIT that states that you will be responsible for a reduced cash price of a maximum of $150 if your insurance denies the claim for the administration of this medication. Last year we had a few insurance companies decline coverage for this service so we had to start this ABN process to ensure patient prices remain affordable. Most insurances are still covering this, but some are not. We are giving you a heads up so that you have the opportunity to call your insurance company and inquire whether this is a covered service and make an educated decision about whether or not you would like to move forward with your appointment. The CPT code you would ask your insurance about coverage for is: 95199.
You cannot start a sublingual therapy if you have open wounds in your mouth, like recently lost teeth, dental procedures, or bleeding sores. If this is the case, please call us back so we can reschedule your appointment.

Consult only Appointments
Please arrive 10 minutes early for check-in.
If you are scheduled for a consult only with one of our providers and do not anticipate or want any allergy testing, you do NOT need to stop antihistamines prior to your visit. In this instance, continue all normal allergy and asthma medications.

Follow Up Appointments for Biologic Medications:
Visits required every 6 months for management of your biologic medication (i.e. Xolair, Dupixent, Fasenra, Nucala, Rhapsido, IVIG, SCIg, etc)
Please arrive 10 minutes early for check-in.
If you are scheduled with one of our providers to follow up about your biologic medications, you do NOT need to stop antihistamines prior to your visit. In this instance, continue all normal allergy and asthma medications.

Venom & Xolair Administration Appointments (Nurse Only)
Please arrive 5-10 minutes early for check-in.
