How to Prepare for Your New Patient or Follow Up Appointment

To help you prepare for your visit, please review the information below.

Please be aware, we are unable to receive text replies. If you have questions about your appointment, need to cancel or reschedule, please call us @ 503-228-0155.

Please ALWAYS bring a current insurance card with you to your visit. Plans are often updated or slightly modified especially at the beginning of a year. We will need to check your card each visit for any changes and scan if needed. Incorrect insurance billing information can cause delays in processing your claim and cause charges to be transferred to you until correct information is obtained. If you are covered by more than one insurance, please bring ALL insurance cards to your visit.  

Additionally, please bring a valid photo ID to your visit.  If the patient is a minor, please have a parent/guardians photo ID available.

  • Bring any questions you have for your provider

  • If this visit is currently scheduled to include allergy testing. Stop antihistamines 5 days and high doses of vitamin C (>1000 mg) prior to this visit. For a complete list of medications to stop or continue, please click here: https://www.allergyclinic.net/medicationsinfo

  • PLEASE NOTE: OHP does not cover skin testing for all conditions.  Currently, OHP covers the cost of allergy testing for asthma and anaphylaxis.  Your provider will discuss with you whether or not OHP is likely to cover skin testing for you.  If skin testing is unlikely to be covered, a cost estimate will be provided and you will have an opportunity to decide if you would like to proceed with skin testing at a cash pay price.
