To open the attached message, you will be asked for a password. Your password is already set based on the information in your medical chart:

How your password is created:
- The first 4 letters of your first name (all lowercase, no spaces or punctuation).
- Followed by your birth month and year in the format MMYYYY.

Examples:
- If your name is John and your birthdate is August 15, 1947, your password is john081947.
- If your name is James and your birthdate is February 2, 2000, your password is jame022000.
- If your name is La’Darius and your birthdate is February 26, 2005, your password is lada022005.
- If your name is just J and your birthdate is March 14, 1997, your password is j031997.

⚠️ Passwords are case-sensitive. Always enter in lowercase.

4. You’ll receive a code by email to complete your registration.

- Use either your username or email address.
- If you forget your login information, click “Forgot username/password?” to reset it.
