How to Prepare for Your SLIT (sublingual immunotherapy) Appointment
To help you prepare for your visit, please review the information below.

Please ALWAYS bring a current insurance card with you to your visit. Plans are often updated or slightly modified especially at the beginning of a year. We will need to check your card each visit for any changes and scan if needed. Incorrect insurance billing information can cause delays in processing your claim and cause charges to be transferred to you until correct information is obtained. If you are covered by more than one insurance, please bring ALL insurance cards to your visit.
Additionally, please bring a valid photo ID to your visit. If the patient is a minor, please have a parent/guardians photo ID available.
Bring an unexpired Epipen. If you need help accessing an affordable epinephrine autoinjector prior to your appointment, please call and talk to one of the clinical staff (503-228-0155)
Please continue your normal allergy and asthma medications (INCLUDING antihistamines if you are taking them). Your allergy symptoms need to be relatively well controlled prior to starting sublingual immunotherapy.
You will need to sign an Advanced Beneficiary Notice (ABN) before the medication will be administered that states you will be responsible for a reduced cash price of $150 IF your insurance denies the claim for the administration of this medication. There are a few insurance companies that are declining to cover this service. This ABN ensures that we can continue to offer this service at an affordable price for patients. Most insurances do cover this service. However, we recommend that you call your insurance company ahead of time and inquire whether this is a covered service so you can make an educated decision about whether or not you would like to move forward with this appointment. The CPT code you need to ask your insurance about is: 95199.
In order to start sublingual immunotherapy, you must not have any open wounds in your mouth such as recently lost teeth, dental procedures, or bleeding sores. If this is the case, please call us as soon as possible so we can reschedule your appointment.
